Requirements for Care Providers
- CNA License in Florida (or HHA certificate – 75 hours minimum)
- Level II Background Screening on file with AHCA
- Valid CPR Card through American Heart Association or American Red Cross
- Free of Communicable Diseases (dated within the last 6 months)
- HIV 2 Hour/AIDS 2 Hour Certificate – 4 Hour
- Alzheimer’s Training Certificate – 2 Hour
- Self-Administration of Medication Certificate – 4 hours (Must provide first 6 hours)
- Professional Liability Insurance – recommended
- Driver’s License
- Social Security Card
- Auto Insurance & Vehicle Registration – You must have your own transportation.
- Level 2 background screening on file with AHCA – State of Florida
- Two work references
- Social Security Card
